Checking In - Civilians

Congratulations on your new position with SUPSHIP Bath.  You are - or soon will be - a member of our talented workforce with the privilege of supporting the building of the Navy’s future Fleet.

On your first day, please come prepared with two forms of identification (e.g. passport, birth certificate, driver’s license etc…)

Although not mandatory, it's helpful for newly hired civilian employees to complete and return the below forms/certificates to their supervisor prior to the start of employment.  Submitting these forms prior to your start date can significantly reduce the time it takes for you to access the SUPSHIP Bath computer network.

Computer Network Access Forms:

E-mail / mail forms to supervisor when completed.

1. SAAR-N Form

2. SOSB User Agreement Form (supervisor-provided)

3. SOSB User Access Form (supervisor-provided)

IT Training Requirements:

Courses can be accessed via http://iase.disa.mil/eta/index.html.  Email course completion certificates to your supervisor or bring them on your first day.

1. Personal Identifiable Information (PII)

2. Phishing

3. Cyber Awareness Challenge

4. Portable Electronic Devices/Removable Storage Media

Security Clearance Form:

All employees are required to be eligible for a security clearance.  Below is a link to the SF-86 Security Clearance Form http://www.opm.gov/investigations/e-qip-application/ . This form will take several hours to complete.  Again, this form does not need to be completed prior to your start date.  However, many new employees find it more convenient to complete this form at home where they have direct access to much of the personal information necessary to complete the process.
