Washington, DC - Washington, DC (April 11, 2022) -- Guest Speaker Diane Vaughan, who holds a Ph.D. in sociology and is a professor at Columbia University, provided her insights into decreasing risk to fix or avoid a problem during the 59th Anniversary of the USS Thresher (SSN 593). USS Thresher was lost at sea on April 10, 1963 with 129 personnel on board. From this tragic event the Submarine Safety (SUBSAFE) Program was established.
