The following New Employee Package contains important forms that are required for all new hires to the Federal government. These forms are necessary in order to properly process your appointment and bring you on board.
Please print and complete the NSWCDD New Employee Package as well as the appropriate State Tax form included in the link.
Selecting the appropriate State Tax form is very important to insure deductions from your pay are allocated to the correct recipients. Please review each form carefully before making your selection. Also be sure to fill out the federal tax form as well. Both a state and federal tax form is needed from each new hire.
Please bring all forms with you to New Employee Orientation on your appropriate start date. Due to privacy issues, please do not email completed forms from this section to your HR Advisor.
Finally, please read the NSWCDD Employee's Guide to the Standards of Conduct, Initial Ethics Training (2011) prior to your reporting date. The guide provides the names and contact information for NSWCDD's ethics counselors if you have any ethics questions after reading the guide. On your reporting day, you will attend a new employee orientation where you will be given an initial briefing on the Standards of Conduct. You will also be given a paper copy of the guide and asked to sign the certificate included as the last page of the guide. Your certification attests to your having read the guide.

Below are forms you may need or might already be familiar with. Please use them accordingly.
This form is used to determine your acceptability for Federal employment and your enrollment status in the Government's life insurance (FEGLI) program.
When completing the form, please ensure you include a detailed explanation for all questions where a "yes" answer is provided. Please note that a "yes" answer to one or more of the questions does not automatically exclude you from consideration for employment. For questions 9-11, please ensure all non-juvenile offenses, including alcohol-related incidents, are listed.
- Upon completion of the form, please sign and date it as an Applicant (Item 17a) and submit it to the Servicing HR Assistant. Please note you will not complete 17b and 18 (if applicable) until you are appointed to a position on your first day of employment.
- If you have prior federal service, you will need to answer question 18 as well.
Employment with the Naval Surface Warfare Center, Dahlgren Division is contingent on the fact that you will be able to obtain and maintain a security clearance. Once completed, the Security Office will be able to conduct a background investigation. Please complete this form and email it within seven (7) business days to dlgr_nswc_cx9_security_office@navy.mil.
PLEASE NOTE: Standard Form (SF) 86 is being revised and a change to Question #21 is being implemented immediately. Please view the Department of Defense Memorandum "Policy Implementation - Mental Health Question, Standard Form (SF) 86, Questionnaire for National Security Positions" for more information. Please answer question #21 based on the information on the third page of the memorandum.
