Please provide the following information:
Who...Employee or service member's full name, rank/grade, title and duty station.
What...Specific wrongdoing and why you believe the activity was misconduct, to include the rule, regulation or law you think they violated.
When...Specific dates and times.
Where...Location where the wrongdoing occurred.
Why and how...Describe why and how you believe the individual perpetrated the offense.
How much...Estimated dollar loss.
What...you have done to try to resolve the issue.
What...you want the IG to do.
REMEMBER: The more detailed information you provide the better we can assist you.
Confidentiality is of the utmost importance. You may choose to remain anonymous or you can choose to be put in a confidential status, which means no one but the Hotline Case Manager will know your name. If you choose not to leave your name remember that details are important and adequate information must be provided to open an investigation. For this reason it would be helpful if you would, at a minimum, provide a phone number where you can be reached.
Following is contact information for the NAVSEA, Navy and DOD hotlines. You may certainly register your complaint through any hotline, but it is recommended you submit your issue through the MARMC hotline. If the subject of your allegation(s) is a MARMC employee, reporting your issue through a higher-level IG hotline does not guarantee a more rapid response; in fact, it will delay the process as hotline complaints are passed along to the Command to which the subject of your allegation(s) belongs.
